JACKSONVILLE, FL (MARKET WIRE) U.S. Care Management has announced that the New York State Health Department has extended its contract to provide community-based disease management services to help address the complex health care needs of select Medicaid recipients with chronic conditions or at risk for a disease in the seven-county area of Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans and Wyoming.
